Child Subsidy Programs
Cream of the Crop works with several child subsidy programs to help make childcare more affordable for parents and families. Follow the links below to the individual programs to see if you are eligible.

ENROLLMENT FORMS
Operational Procedure
Contains our hours of operation and the days during the year that we are closed.
Digital Images Release
Gives Cream of the Crop permission to use your child's picture/video on social media, online, or in print.
Sick Policy
To ensure the health and safety of all of our students and staff, please adhere to our sick policy.

MEDICINE/ALLERGY INFO
Allergy Care Plan
Must be filled out and signed by a doctor if your child has any allergies related to food or environmental factors.
Medication Administration
Please fill out the top section completely.
All medication MUST be turned into the office!!!
If any child requires medication only once per day, we ask that the parents administer this medication at home. Only medication that needs to be given multiple times during the day (antibiotics, breathing treatments, etc) will be accepted.
ADMINISTRATION
Vacation Credit
Each family receives a week of tuition vacation each calendar year. Please contact office or see Parent Handbook for more information.
Release Authorization
Gives COTC Authorization to release your child/children to someone not already on your authorized pick up list.
Parent Handbook
Contains all of the information regarding COTC, our policies, procedures, information about our classrooms, and our contact information.
Referral Credit
If you refer a family to our center, we will credit you $25 on your account after one month's enrollment. This is applicable to as many families that enroll. In other words, if you refer 4 families to us, you get $100 off one week's tuition!!
Fill out the form with your contact information and the form that you are needing as the Subject and one of our office staff will email it to you during regular business hours.
